📝 NEW PATIENT INTAKE FORM (REQUIRED)

Once you book your initial appointment, you will receive an electronic intake form sent to the email provided. This form must be completed prior to your appointment. Completing it ahead of time allows me to fully prepare for your session and provide you with the best possible care. If you do not receive the email, please contact me right away at 226-347-8431.

✨ WAITLIST

Please join the waitlist incase of cancellations! Cancellations and last-minute changes happen often — you’ll get an automatic notification when something opens up.

⏳ CANCELLATION & RESCHEDULING POLICY

Your appointment time is reserved just for you. Out of respect for both my time and other patients who may be waiting for care: Please provide a minimum of 24 hours’ notice for cancellations or rescheduling. Less than 24 hours’ notice may result in a $50 cancellation fee. No-Show appointments will be charged the full price of the missed appointment. Late cancellations and no-shows prevent another patient from receiving care during that time, so I truly appreciate your understanding and cooperation.
